About GB-21 Shoulder Well

GB-21 Jianjing is the point that virtually everyone discovers intuitively -- it is the spot on top of the shoulder that begs to be squeezed when tension mounts. The thick trapezius muscle at this location is the body's primary repository for stress-related tension, and Jianjing is its release valve. The name 'Shoulder Well' describes the deep, well-like depression that can be felt in the muscle when it is not bound up with tension, and the therapeutic goal is to restore that openness.

As a meeting point of the Gallbladder, Triple Burner, and Stomach meridians with the Yang Wei Mai (Yang Linking Vessel), Jianjing sits at a crossroads of major energetic pathways. Its strong descending action draws qi downward from the head and shoulders -- relieving the upward accumulation that produces tension headaches, stiff neck, and the sensation of carrying the world's weight on the shoulders. This descending action is also why the point promotes labor and lactation (both requiring downward movement of qi) and why it is strictly avoided during pregnancy.

In modern clinical practice, GB-21 is indispensable for the ubiquitous pattern of desk-related neck and shoulder tension. Hours of computer work, phone use, and emotional stress all cause the trapezius to contract and the shoulders to rise toward the ears. Firm acupressure at Jianjing, combined with gentle neck stretches and shoulder rolls, is one of the most immediately relieving self-care interventions available.

How to Find This Point

Find the most prominent vertebra at the base of the neck (C7) and the bony tip of the shoulder (acromion). The point is exactly halfway between these two landmarks, at the top of the shoulder where the trapezius muscle is thickest. This is the spot most people instinctively grab when their shoulders are tense.

Technique

Press firmly downward with the thumb, middle finger, or use a pinching grip with the thumb and fingers on the trapezius muscle. Deep sustained pressure, kneading, or rhythmic squeezing. This point tolerates and often requires vigorous stimulation.

Pressure & Duration

Firm to deep pressure for 2-3 minutes per side.

Benefits

Relaxes the sinews, descends qi, promotes lactation, benefits the shoulder, activates the channel, promotes labor

Indications

Neck and shoulder tension, stiff neck, headache from shoulder tension, difficulty turning the head, shoulder pain, arm pain, mastitis, insufficient lactation, difficult labor

TCM Actions

Relaxes the sinews, activates the channel, descends qi, promotes lactation, promotes labor, meeting point of the Gallbladder, Triple Burner, and Stomach meridians with the Yang Wei Mai

Self-Care Use

The go-to point for shoulder and neck tension. Grab the top of your shoulder with the opposite hand and squeeze firmly, pressing the thumb into the thickest part of the trapezius muscle. Hold and knead for 2-3 minutes per side. This is one of the most satisfying points to press for stress-related tension. Also useful for headaches that originate from shoulder tightness. DO NOT USE DURING PREGNANCY.

Contraindications & Cautions

CONTRAINDICATED DURING PREGNANCY -- this point has a strong descending and labor-promoting action. Avoid in individuals with severe hypotension (can cause dizziness from its descending action). Use caution in individuals prone to fainting.
